Dto types should use UriComponents as the actual URI class won't be recreated automatically

This caused compile problems in another PR I was working on
Add IChatTerminalToolInvocationData2 and pass along richer commandLine
Pull from actual xterm instance instead of creating new one
`renderStringAsPlaintext` just called `renderMarkdownAsPlaintext`. I think it makes sense to combine them
Combine related md stripping functions
For MCP I can already kind of get this from the tool name, but for
extension tools we previously had no way to know which extension
provided the tool.

This PR adds a `LanguageModelToolInformation.source` containing details
about where the tool came from. I also use this to include the MCP
server instructions which is needed for #250017.

- Use clearer `setSetInnerHtml` name
- Update markdown renderer to also specify `override`
- Indicated that `hooks` should not be used and will be replaced
