Opt-out of year end gifts

Provide a check box that defaults to On that is recording that the donor/member does indeed want a year end gift. Wording could be "Yes, please, provide me with a year end gift."

@energydev Do you think we should allow people to opt-out before they (or even we) know what the gift is? I was imagining that near the end of the year we'd send folks an email saying "Hey, for your generous contribution we'd like to send you ______. In order to get your gift, please click the link below and let us know what address to send it to. You have until #{cut_off_date} to claim your gift."

@jagthedrummer allowing an opt-in at the end of year works as well. We will definitely need a way to track their donations in combination with any gifts they receive so that tax receipts can be more easily generated.
