Adding an issue template

Just saw that y'all didn't have an issue template in place with validations. So I made an issue template that you might be able to copy over and use as your own. You can check it out here:

Follow up on this issue, I'm trying to get a list of starters together for design systems. Is the design system kit the right one to use? Are their simpler versions of it maybe?
