Always capture start/end time in Instrumenter?

Currently the Instrumenter defers to the SDK for default start/end time, but this creates some duplication when capturing the same times for metrics.

It seems like it could simplify things (and reduce duplication) to provide a default StartTimeExtractor/EndTimeExtractor.
