[enhancement] more intelligent way to disconnect

currently there is a hard-coded delay after calling disconnect and before the ca_clear_channel is actually called.

This is a dirty workaround of the deadlock occasionally encountered when ca_create_create and ca_clear_channel are called at virtually the same time.

A better solution is indeed needed.
