Bundled Rates

I see instances of in network object not having negotiation arrangement as bundled even when the data suggests it is. For example billing code S2140 (Cord blood harvesting for transplantation, allogeneic) having billing code modifier 47 (Anesthesia by Surgeon) suggesting anesthesia was part of the billed rate. Is this industry standard or am I missing something here?
My primary concern is how to correctly identify and tag the negotiates rate to be bundled when it is not explicitly stated?
